If you’re concerned that you may be missing a patient questionnaire, please get in touch. Please include the patient’s surname and the approximate date it was completed, plus the reference number if known.
Every patient who completes a Pre-Op questionnaire is shown a reference number when the form is submitted (plus they will get emailed this reference number if they include an email address in their questionnaire). If a patient advises that they’ve filled in your form and you haven’t received it, it could be that they’re confused and are thinking of the hospital form. It’s worth clarifying this with them and asking for the reference number they received.
