Please include the patient’s surname and the approximate time it was completed. We’ll check our system logs to see what happened.
Occasionally, we receive unallocated questionnaires that haven’t come through the correct page. If you’ve received other questionnaires with the same surgeon, or on the same operation day, we’ll usually contact you within 24 hours to check if it’s yours. Please feel free to contact us sooner.
If a patient advises that they’ve filled in your form and you haven’t received it, it could be that they’re confused and have completed the hospital form, thinking you will receive it. It’s worth clarifying this with them – we have seen this happen a few times.
