Demo – Pre-Op Health Questionnaire
Please Note: This form is a demo pre-op health questionnaire, for testing purposes only. If you are a patient please click here to find your Anaesthetist to fill in their individual form.
To begin:
– Enter your real name and email address below and click “Start the Demo” – You will then be shown our Standard Pre-Op Questionnaire
– Please enter a date of birth to see either the Adult version or the Paediatric version. Adult questions are shown for those 14+. To see the full list of questions for children, enter an age of 2 or under.
– Fill in the questionnaire and click “Submit – Send me the demo” at the bottom – you can then check your email to see how it arrives in your inbox (+ check the patient’s email address you entered, to see what they see as well)
Please keep in mind that it is a clever questionnaire – if you click Yes to questions you may see more questions appear.
This Standard questionnaire comes with all Standard Plans. An IFC and a Non-IFC option are available.
