About Dr Samuel Cho
Dr Samuel Cho is an Australian doctor who is officially recognised as a specialist in anaesthesia, holding a Fellowship with the Australian and New Zealand College of Anaesthetists. After qualifying, Dr Cho attained further experience overseas in a fellowship In Ottawa, Canada.

What do you need to do?
To provide you with the safest and most suitable anaesthetic, it is essential that Dr Cho has your full medical and surgical history. In order to assist with this, Dr Cho requests that all patients complete the Pre-Op Questionnaire.

Please remember to fast from solids 6 hours prior to your appointment. You may have clear fluids (water, apple juice, black tea/coffee etc) up until 2 hours prior to your appointment.

After your procedure Dr Cho would appreciate you taking just a few minutes to fill out the Patient Experience Survey. Your answers are completely anonymous and will help Dr Cho greatly in improving care for patients in the future.
