He is a Fellow of the Australian and New Zealand College of Anaesthetists.

What do you need to do?
Not all anaesthetics and surgeries are the same. Therefore, to provide you with the safest and most effective anaesthetic care, it is very useful to have your full medical and surgical history. In order to assist with this, Dr Bond requests that all patients complete the Pre-Op Questionnaire. This will also allow him to help ensure your medications and perioperative care are optimised and the chances of last minute changes minimised.

You may receive a similar questionnaire from your admitting hospital, however this information is not necessarily received by Dr Bond in a timely fashion to plan your medications etc perioperatively.
